Texas Health Physicians Group North Texas is seeking to hire a licensed Care Manager Registered Nurse to join the Care Coordination team.

The work schedule will be Monday - Friday, 8:00am - 5:00pm. The Care Manager will work each day from one of their PCP Clinics, Richardson, Addison, White Rock and Presbyterian Hospital Dallas with expectation to travel to one each day. Driving into our Northpoint office once or twice per month for meetings.

The Care Manager is responsible for managing THPG's primary care population for high risk and/or chronic illness patients via creation of pro-active care plans promoting effective education of red flags signs/symptoms and medications as well as self-management to improve patient engagement and adherence to PCP's plan of care by facilitating access and timely healthcare delivery to achieve cost effective, optimal quality, and financial outcomes.

Minimum Education:
Graduate of a NLN (National League of Nursing) or CCNE (Commission on Collegiate Nursing Education) accredited nursing programs.

Associate Degree; BSN preferred.

RNs hired after January 2017 require completion of BSN within 2 years of start date.

RN - Registered Nurse Completion of RN from an
accredited program. Must be licensed in the state of Texas and be in good standing Upon Hire
DL - Drivers License Must be valid Upon Hire

Required Experience:
3 years' in care coordination, ambulatory resource management, or discharge planning with managed care population. Skilled nursing facility/home health experience preferred.

Complete BioPsychoSocial assessments in PCP EMR identifying and addressing barriers to PCP plan of care adherence with focus on gaps in care and patient goals.

Conduct patient interviews via clinic face-to-face encounters and/or telephonic contacts documenting interventions and outcomes in PCP EMR.

• Practiced in and knowledge of Medicare Advantage and Commercial programs and benefits preferred. • Chronic disease management of diverse patient populations and/or experience in health coaching. • Excellent time management skills with ability to prioritize tasks effectively and efficiently.
